Contraindications

Contraindication:

• For patients who are contraindicated to bedaquiline and/or linezolid, Pretomanid tablets should not be used in combination with bedaquiline and linezolid.

Warning:

• Adverse liver reactions have been reported with Pretomanid tablets, bedaquiline and linezolid. Monitor symptoms, signs, and results of liver-related laboratory tests. If there are signs of liver damage, the entire treatment regimen should be interrupted.

• Combination regimens with Pretomanid tablets, bedaquiline and linezolid have been reported to result in myelosuppression. Monitor the complete blood count. In case of severe myelosuppression or worsening, the dose of linezolid should be reduced or interrupted.

• Combination regimens with Pretomanid tablets, bedaquiline and linezolid have been reported to cause peripheral neuropathy and optic neuropathy. Monitor visual function. In case of visual impairment symptoms, an ophthalmological assessment should be performed. In case of neuropathy or worsening, the dose of linezolid should be reduced or interrupted.

• Combination regimens with Pretomanid tablets, bedaquiline and linezolid have been reported to result in prolonged QT intervals. Combination with drugs that prolong the QT interval may result in prolongation of the QT interval. Monitor the ECG. In case of severe ventricular arrhythmias or prolongation of the QTcF interval beyond 500 msec, discontinue the combination of Pretomanid tablets, bedaquiline and linezolid.

• Reproductive effects: Pretomanid causes testicular atrophy in male rats and affects fertility. Please inform the patient that the reproductive toxicity observed in animal studies and its potential effects on human male fertility have not been adequately evaluated.

• The combination of Pretomanid tablets, bedaquiline and linezolid has been reported to cause lactic acidosis. In case of severe lactic acidosis, consider interrupting linezolid or discontinuing the entire combination of Pretomanid tablets, bedaquiline and linezolid.

Medication risks

AEs:

• The most common adverse effects (≥ 10%) include peripheral neuropathy, acne, anemia, nausea, vomiting, headache, elevated aminotransferase, dyspepsia, decreased appetite, rash, itching, abdominal pain, pleural pain, elevated gamma-glutamyltransferase, lower respiratory tract infection, hyperamylaseemia, hemoptysis, back pain, cough, visual impairment, hypoglycemia, abnormal weight loss, and diarrhea.
